Social Worker (Program Coordinator) Care Coordination and Integrated Case Management
What you'd do
The Care Coordination & Integrated Case Management (CCICM) Social Work Program Coordinator, in collaboration with a Nursing CCICM counterpart, is responsible for implementation, coordination, and management of all administrative and clinical aspects of the CCICM framework. The CCICM framework is a specialized, collaborative practice among multiple healthcare professions which provides structure and standards to support inter-professional collaboration throughout the continuum of care.
Major duties
Total Rewards of a Allied Health Professional Duties for this position include, but are not limited to: Serve as Consultants and Subject Matter Experts on CM practice, including keeping abreast of VA policies, laws, and regulations that affect CM delivery to Veterans, their families, and caregivers. Collaborate with internal stakeholders to assess CM composition, structure, and services to identify promising practices and gaps in CM services and identifying opportunities to enhance communication, collaboration, and coordination across the care continuum. Guide medical center through the CCICM sustainment and develop/review annual CCICM Education Plan. Coordinate and collaborate with the facility level care coordination, care management, and case management teams to include but not limited to nurse and social worker navigators, care coordinators, care managers, and case managers. Develop procedures and processes to support cost-effective, high-quality CM across the VA medical facility to eliminate duplication of services where appropriate. Track trends of performance metrics reporting, monitoring quality of documentation and workload productivity and validate coding and labor mappings. Advocate on behalf of care coordination and case management services and facilitate communication with facility executive leadership on care coordination and care management in case management strengths and challenges. Facilitate staff access to nationally recognized care coordination and case management standards of practice to ensure delivery of professional care coordination, care management and case management services across the healthcare continuum. Work Schedule: Monday-Friday 0730-1600, subject to change based on the needs of the facility. Relocation Incentive (Sign-on Bonus): Not Authorized. What you need to qualify
Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). NOTE: Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Note: Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12 (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice Preferred Experience: The ability and experience to contribute to the program's resource and fiscal management and experience in the administration of a clinical program and responsibility for the clinical programming and the ability to prepare reports and statistics for facility, VISN, and national use. Experience providing leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. Experience to initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing, and implementing ideas for improvement and evaluating efficacy of improvement efforts. The actual grade at which an applicant may be selected for this vacancy is GS-12. May require standing, lifting, carrying, sitting, stooping, bending, puling, and pushing. May be required to wear personal protective equipment and undergo annual TB screening or testing as conditions of employment. Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated, and ventilated. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations.
