Social Worker (Program Coordinator)
What you'd do
The Inpatient Complex Care Social Worker Coordinator, is assigned to the Acute Care Social Work Program in Social Work Service at the New Mexico VA Health Care System (NMVAHCS). The Acute Care Social Work Program includes two general program areas (1) Inpatient Medicine, Surgery, Mental Health and (2) Outpatient Medical Specialties such as Renal/Dialysis, Hematology/Oncology, Emergency Department, Neurology, Complex Care Coordinator and Pain Management.
Major duties
Duties include not limited to: 1. Performs program coordination duties as appropriate and/or requested by supervisor, Chief, Assistant Chief of Social Work Service, or designee. These may include but are not limited to the following: a. Oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. b. May be responsible for developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice, and service delivery. c. Provides analysis and evaluation of clinical program data and computerized programs to identify systemwide trends and needs to enhance the quality of service. d. May be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. e. Administratively responsible for clinical programming and prepares reports and statistics for facility, VISN, and national use, including case reviews for NMVAHCS Complex Discharge Action Advisory Group (CDAAG). f. Provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff, and is responsible for training new employees assigned to the inpatient medical, surgery, and mental health units. g. Initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. h. Oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. i. Develops, implements, evaluates and reports on program performance, including assigned measures, monitors, data and other metrics of interest. 2. Incumbent provides direct care services. Provides independent and expert clinical psychosocial and case management services in a specialized area of practice to Veterans who tend to have serious and severe crisis's, may lack any familial and community support, may be poor self-monitors, may frequently fail to comply with instructions or treatment, or may have major deficits in coping skills and require continuing professional psychological support. Anticipated workload assignment is 70% of time. 3. The incumbent provides clinical supervision for licensure or certification. 4. The incumbent represents the program in contacts with other agencies and the public. 5. Explains veteran's treatment and progress to third parties as appropriate. Manages the quality control and evaluation of the professional services provided. Devises methods of obtaining data and evaluates data to determine the effectiveness of treatment. Identifies consistent qualities of successful treatment and of failed treatment. 6. The incumbent coordinates team reviews of medical records and assures that the documentation of the assigned staff reflects the services provided and meets requirements of JCAHO, VHA, Medical Center, and Social Work Service documentation policies and standards. 7. The incumbent ensures that clinical care within the assigned program meets the standards prescribed by local codes and is within the deliver care standards and guidelines in accordance with applicable grant and contract proposals, as appropriate. The incumbent intervenes to facilitate the program compliance of the program proposal or contract guidelines where appropriate. 8. Networks with community agencies to manage appropriate Veteran referrals. 9. The incumbent takes responsibility for improving professional skills, participating in planning, and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. 10. Incumbent maintains productivity at the established required level. Work Schedule: Monday - Friday/ 8:30 AM - 4:30 PM Pay: Competitive salary and regular salary increases. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. What you need to qualify
Applicants pending the completion of educational or certification/licensure requirements may be referred to and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Licensure: Candidate must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Hospice and Palliative Care, Geriatric Social Work References: VA Handbook 5005, Part II, Appendix G39 - Social Worker Qualification Standard. The full performance level of this vacancy is GS-12. Physical Requirements: Work is sedentary but also demands standing, walking, bending, twisting, and carrying light items.
