Social Worker (Intake/Aftercare)
What you'd do
The Intake/Aftercare Coordinator provides assessments, intake, discharge planning, aftercare, and clinical social work services for American Indian and Alaska Native youth ages 12-17 in residential treatment for substance use and co-occurring conditions. The position works with Tribal programs, families, referral sources, and treatment teams to support continuity of care and reports to the Clinical Director or designee.
Major duties
Coordinates referrals for youth requiring residential behavioral health treatment, including substance use disorders and co-occurring behavioral health conditions, ensuring timely admission and continuity of care. Coordinates the intake and admission process by reviewing referrals, ensuring required documentation is complete, communicating with referral sources, families, Tribal programs, and interdisciplinary treatment team members, and facilitating timely admission, treatment planning, discharge planning, and aftercare coordination. Conducts comprehensive biopsychosocial and behavioral health assessments to evaluate substance use disorders, co-occurring behavioral health conditions, psychosocial functioning, strengths, risk factors, and treatment needs. Provides clinical assessment, triage, consultation, crisis intervention, and care coordination for youth and families presenting with substance use disorders, co-occurring behavioral health conditions, and related psychosocial needs. Develops individualized discharge and aftercare plans, coordinates continuity of care with Tribal and community providers, facilitates referrals for ongoing behavioral health services, and conducts post-discharge follow-up to support successful community reintegration. Develops and maintains collaborative relationships with Tribal governments, Urban Indian Organizations, behavioral health providers, schools, courts, and community partners to support outreach, education, referral development, and continuity of care. To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Degree: social work. Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education. Master's degree with practicum: Applicants who complete all of the requirements for a master's degree in social work that included a practicum are eligible for GS-9. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS:GS-9: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-7 grade level in the Federal service. This experience may be obtained in either the private or public sector performing the following type of work and/or tasks: Conduct psychosocial assessments; assist patients and families in accessing behavioral health, Tribal, community, state, and federal resources; coordinate referrals and continuity of care; participate in interdisciplinary treatment planning; provide case management, discharge planning, and aftercare coordination; document clinical services; and assess patient needs to support appropriate behavioral health treatment and community reintegration. OR 2 years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree. OR Combination of Graduate Education and Professional Experience: Combinations of successfully completed graduate level education and specialized experience may be used to meet total experience requirements. GS-11: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-9 grade level in the Federal service. This experience may be obtained in either the private or public sector performing the following type of work and/or tasks: Conduct comprehensive psychosocial assessments; coordinate intake, discharge, and aftercare services; develop individualized treatment and transition plans; provide behavioral health case management, crisis intervention, care coordination, family engagement, and consultation; collaborate with interdisciplinary treatment teams and community partners; and coordinate referrals to behavioral health and community resources for individuals with substance use disorders and co-occurring behavioral health conditions. OR 3 years of progressively higher-level graduate education leading to a Ph.D. degree or Ph.D. or equivalent doctoral degree. OR Combination of Graduate Education and Professional Experience: Combinations of successfully completed graduate level education and specialized experience may be used to meet total experience requirements. GS-11 Selective Factor: Applicants selected at the GS-11 full performance level must possess a current, unrestricted license issued by a U.S. state, the District of Columbia, Puerto Rico, or a U.S. territory that authorizes independent clinical social work practice (e.g., LCSW, LICSW, LISW, CISW, or equivalent). Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
